At Park Avenue Acoustics, Inc., our Doctors of Audiology are dedicated to meeting your hearing healthcare needs.
We are conveniently located in Midtown on Madison Avenue between 39^th & 40th. We understand the new hearing
requirements as well as your benefits for the Department of Transportation. We will work with your existing hearing
condition to meet those requirements and improve your quality of life. We are here to help people with hearing
deficiencies live the life they want with the hearing they have. For your convenience:

We are a UHC participating provider and we will complete all Metro North Hearing Aid Reimbursement forms
for prompt payment. All forms are on file in office.

If you are removed from service because you failed your hearing test immediate appointments can be made.

Evening hours can be accommodated.

Financing available.

We will contact the Metro North medical department on the members behalf to get them back in service as
soon as possible.

We are aware of the Federal Railroad Administration's minimum hearing requirements for railroad employees.
